Dr. Phil Hellman MD is the medical director for purelyIV and the owner of Paradox Health Direct Primary Care in Rochester, MI. He graduated from Medical school at the Michigan State University College of Human Medicine in 2013 and completed a residency in Family Medicine at North Colorado Family Medicine in Greeley, CO in 2016. Dr. Hellman and his wife Chelsea stayed out west for the first few years after residency, accepting a position in Silverton, OR with a large family medical group. There he practiced rural family medicine - delivery babies, doing C-sections, endoscopy, and of course, bread and butter family medicine.
In 2018, Dr. Hellman decided to move back home and start his own independent medical practice. The vision for this was born out of his focus is on patient care and the nuance involved in creating truly individualized plans for the particular needs of each person. In 2023, he and Erin met and decided there was an opportunity in the community for at-home IVs. This is when purelyIV was hatched. Dr. Hellman's focus on patient care and intellectual curiosity towards problem solving have helped to accelerate the growth and sustainability of purelyIV in the community. His vision for purelyIV is to both be a successful company, as well as a way to strengthen the surrounding community by providing a scarce resource at an affordable price.
Lead Practitioner

Erin Boumansour, MSN, ACNP-BC, is the Lead Practitioner for purelyIV. She brings more than 20 years of nursing experience and over a decade of experience as a nurse practitioner caring for patients across a wide range of healthcare settings.
Erin began her career as a Registered Nurse in the fast-paced emergency department at Bellevue Hospital in New York City. She later earned her Master of Science in Nursing from the University of Pennsylvania, where she completed her Adult Acute Care Nurse Practitioner training with a minor in forensic nursing.
Following graduate training, Erin practiced as a Nurse Practitioner at Memorial Sloan Kettering Cancer Center, where she cared for medically complex patients in a critical care environment at one of the nation’s leading academic cancer centers.
Her clinical background spans emergency medicine, critical care, oncology, palliative care, hospice, and forensic nursing, where she served as a Sexual Assault Nurse Examiner supporting patients through sensitive and complex situations.
At purelyIV, Erin oversees clinical protocols, provider training, and the development of new wellness services within the practice.
She is also certified in hormone replacement therapy and has a particular interest in menopause care, metabolic health, and personalized wellness medicine. Erin is passionate about helping patients better understand their health and develop thoughtful, individualized treatment plans that support long-term vitality and well-being.
In addition to her clinical work, Erin has served as a nursing instructor for undergraduate and graduate students and remains committed to education and advancing high-quality patient care.

As the CEO of purelyIV, Michael Boumansour brings over three decades of experience in software development, business administration, and leadership to the company. He oversees all non-medical aspects of the business, including finance, operations, and digital marketing, ensuring the smooth execution of day-to-day activities and long-term strategic goals. Michael's deep understanding of technology and entrepreneurship has been instrumental in purelyIV's growth since its inception. His commitment to offering personalized, high-quality service sets purelyIV apart in the competitive mobile IV therapy space.
Michael's leadership approach combines his technical expertise with a passion for innovation, always seeking new ways to improve the customer experience. Prior to purelyIV, Michael held various senior positions in software development, where he successfully led projects that spanned a wide range of industries. His problem-solving skills and drive for excellence are key to purelyIV's mission of providing superior IV therapy services in the Metro Detroit area.

Nurse Practitioner
Rachel Nelson is a board-certified Adult-Gerontology Primary Care Nurse Practitioner with more than two decades of nursing experience and over seven years of experience as a nurse practitioner. She earned her Master of Science in Nursing from Madonna University and her Bachelor of Science in Nursing from University of Michigan.
Rachel has spent much of her career caring for patients with complex medical conditions in community-based settings, providing comprehensive evaluation, symptom management, and ongoing clinical support. She works closely with interdisciplinary healthcare teams to ensure patients receive thoughtful, coordinated care tailored to their individual needs.
In addition to her work with patients managing chronic and serious illnesses, Rachel has developed clinical experience supporting women through menopause and hormonal changes. She enjoys helping patients better understand their bodies and working collaboratively to develop personalized treatment plans that support overall health and well-being.
Rachel is also board certified as an Advanced Certified Hospice and Palliative Nurse (ACHPN), a specialty certification held by a small number of nurse practitioners nationwide. She is known for her thoughtful clinical approach and her ability to build strong, trusting relationships with patients and families.
Nurse Practitioner
Mary Krzystowczyk is a board-certified Adult-Gerontology Nurse Practitioner with more than a decade of nursing experience in hospital and community-based healthcare settings. She earned her Master of Science in Nursing from Spring Arbor University and her Bachelor of Science in Nursing from Michigan State University.
Mary began her nursing career at Munson Medical Center in Traverse City, where she worked as a charge nurse on an orthopedic unit caring for patients recovering from surgery while also supporting and coordinating care among the nursing team.
She later transitioned into nurse practitioner roles providing hospice and palliative care, where she performed comprehensive assessments, supported patients with complex medical conditions, and guided families through important healthcare decisions. This experience shaped her thoughtful and compassionate approach to patient care.
Mary also has a growing clinical interest in menopause and women’s health and is currently pursuing additional education in hormone therapy.
Mary is known for her calm presence, attentive listening, and genuine compassion for the patients and families she serves.
Nurse Practitioner
Jill Trudell is a Registered Nurse with approximately five years of experience in hospital and surgical nursing. She earned her Bachelor of Science in Nursing from Oakland University.
Jill began her nursing career at Beaumont Hospital, Royal Oak, where she worked in a surgical progressive care unit caring for patients recovering from complex surgical procedures. In this role, she developed strong skills in patient assessment, monitoring, and post-operative recovery while working closely with multidisciplinary care teams.
She later transitioned into perioperative nursing, gaining experience supporting patients before and after surgery in a procedural setting.
Jill currently focuses her clinical work with purelyIV and is pursuing her Nurse Practitioner degree as she continues to expand her clinical training and expertise.

Shanelle Cross is a Registered Nurse with more than 18 years of clinical experience across hospital and surgical care settings. She earned her Associate Degree in Nursing from Wayne County Community College and later completed a Master’s degree in Health Care Administration from Walden University.
Early in her career, Shanelle worked at Henry Ford Hospital in Detroit, where she cared for critically ill patients in a high-acuity environment and developed a strong foundation in clinical assessment and patient care.
Shanelle later transitioned into surgical nursing at a high-volume outpatient surgical center, where she gained extensive experience caring for patients before and after procedures. She currently serves as the Clinical Director at the center, where she helps oversee clinical operations and supports nursing staff in delivering safe, high-quality patient care.
Shanelle brings nearly two decades of hands-on clinical experience and a calm, thoughtful approach to patient care.
Registered Nurse
Jamie Haksluoto is a Registered Nurse with approximately seven years of experience in hospital and procedural nursing settings. She earned her Bachelor of Science in Nursing from Wayne State University after previously completing her medical assistant training at Macomb Community College.
Jamie began her healthcare career working in hospital medical-surgical units before transitioning into registered nursing roles across several hospital campuses in Southeast Michigan. Her experience includes caring for patients in progressive care and procedural settings while supporting multidisciplinary care teams.
She currently works in endoscopy nursing, where she provides care for patients before and after procedures in a hospital-based setting.
Jamie is known for her attention to detail and her ability to create a supportive and reassuring environment for patients.
Registered Nurse
Kenneth is a registered nurse with a Bachelor of Science in Nursing from Eastern Michigan University and more than a decade of hands-on clinical experience. His background includes psychiatric emergency care, adult inpatient nursing, surgical and rapid response nursing, cardiac step-down care, utilization review, and clinical instruction for nursing students. His experience with assessment, medication administration, de-escalation, patient education, and interdisciplinary care supports the thoughtful, professional visit experience expected from purelyIV.
Registered Nurse
Makenna Rottman is a Registered Nurse with approximately five years of experience in pediatric and maternal-child health nursing. She earned her Bachelor of Science in Nursing from Saginaw Valley State University, graduating Cum Laude.
Makenna began her nursing career at Children’s Hospital of Michigan, where she worked as a pediatric float nurse caring for patients across a variety of hospital units. This experience provided her with a broad clinical foundation while working in a fast-paced pediatric hospital environment.
She currently works as a maternal-child health clinical float nurse at Corewell Health Troy Hospital, caring for patients across pediatric, neonatal, and postpartum units.
Makenna is also pursuing her Nurse Practitioner degree as she continues to advance her clinical education and training.

Jennifer Cudworth is a Registered Nurse with over ten years of experience in emergency medicine, behavioral health, and forensic nursing. She earned her Associate Degree in Nursing from Jackson College and later completed her Bachelor of Science in Nursing from Eastern Michigan University.
Jennifer began her nursing career in the Emergency Department at Henry Ford Allegiance Health, where she cared for patients with a wide range of acute medical conditions in a fast-paced hospital environment.
In addition to her emergency nursing experience, Jennifer has worked extensively in behavioral health and psychiatric emergency services. She also serves as a Sexual Assault Nurse Examiner (SANE), providing specialized care and support to patients during sensitive and complex situations.
Jennifer is known for her strong clinical assessment skills and her compassionate, patient-centered approach to care.

Jena McBride is a Registered Nurse with over seven years of experience in hospital and surgical nursing settings. She earned her Bachelor of Science in Nursing from Michigan State University and has cared for patients across a range of high-acuity hospital environments.
Earlier in her career, Jena worked in the Medical Intensive Care Unit at Beaumont Hospital, Royal Oak, a Level I trauma center, where she gained extensive experience caring for critically ill patients.
She currently works in the Intensive Care Unit at Henry Ford Rochester Hospital and also has experience in surgical and procedural care settings, including pre-operative and post-anesthesia care.
Jena is known for her strong hospital training and her ability to remain calm and attentive in complex clinical situations.

Kim Fugate is a Registered Nurse with over eight years of experience in emergency and surgical nursing. She earned her Associate Degree in Nursing from Oakland Community College before completing her Bachelor of Science in Nursing from Oakland University. Earlier in her healthcare career, she also trained and worked as an Emergency Medical Technician (EMT), developing a strong foundation in emergency patient care.
Kim began her nursing career in the Emergency Department at Detroit Receiving Hospital, a Level I trauma center, where she cared for patients with a wide range of acute medical and traumatic conditions in a fast-paced environment. She later continued her emergency nursing practice at Huron Valley-Sinai Hospital, further strengthening her skills in patient assessment and clinical decision-making.
She later transitioned into perioperative nursing, gaining experience caring for patients before and after surgical procedures.
Kim now focuses her clinical work with purelyIV while helping support the continued growth of the practice. Her background in emergency and surgical nursing has shaped her ability to remain organized and attentive while caring for patients.
Registered Nurse
Savannah Piper is a Registered Nurse with over seven years of experience across critical care, home health, and surgical nursing settings. She earned her Bachelor of Science in Nursing from Oakland University.
Savannah began her nursing career at Henry Ford Health System in West Bloomfield, where she cared for patients in a neurosurgical intensive care setting and developed strong skills in patient assessment, IV access, monitoring, and responding quickly to changing clinical conditions.
She later expanded her experience into home health nursing, providing care for patients in their homes, including wound care, medication reconciliation, phlebotomy, and patient and family education. She currently works in orthopedic surgical nursing, caring for patients before and after procedures in an ambulatory surgery setting.
Savannah is known for her attention to detail, calm presence, and commitment to helping patients feel safe, informed, and well cared for throughout their experience.
Operations
Wendy is the friendly voice behind the scenes helping to keep purelyIV running smoothly. As our virtual front desk coordinator, she's often the first point of contact for clients, answering questions, assisting with scheduling, and making sure every experience with us is seamless and stress-free. After raising ten children over nearly three decades, Wendy re-entered the workforce in 2019 and quickly became known for her reliability, calm presence, and genuine care for others. Her attention to detail and warm, reassuring communication make her an essential part of the purelyIV team.
